NEWPORT BEACH, Calif.–(BUSINESS WIRE)–Connected Care (www.cchealth.com/), the world’s first global Payvidance™ platform, announced one of its subsidiaries — Excelera ACO — was selected by The Centers for Medicare & Medicaid Services (CMS) to participate in a Medicare Shared Savings Program as an accountable care organization (ACO), basic track A model. The initiative launched July 1, 2019.
Excelera ACO brings providers of services, provider groups and suppliers together into an ACO to promote accountability and coordination for a population of Fee-For-Service Medicare beneficiaries. Excelera ACO moves away from volume-based to value-based care delivery by promoting coordination of care, high-quality care, efficient service delivery and patient-centricity as the only long-term, sustainable solution.
